About the Role

Key Responsibilities

• Develop and implement occupational therapy treatment plans based on physician orders and patient assessments.
• Provide interventions to improve motor skills, transfers, coordination, strength, and the ability to perform ADLs such as feeding, grooming, bathing, dressing, and toileting.
• Continuously assess and monitor the patient’s response to treatment during each visit and adjust the care plan accordingly for maximum benefit.
• Educate patients and their families or caregivers on home programs, safety strategies, and their role in achieving therapeutic goals during and after the course of care.
• Collaborate with physicians, other therapists, nurses, and healthcare providers to evaluate patient needs and implement appropriate interventions.
• Review physician referrals, medical history, and documentation from past treatments to inform therapy planning and goals.
• Accurately and promptly document patient visits, progress, and outcomes using the agency’s online management or EMR system.
• Educate patients and families on what to expect during recovery, including coping with injury or illness, adaptive equipment, and lifestyle modifications to promote independence.

Requirements

• Graduate of an accredited Occupational Therapy program (Master’s or Doctorate level).
• Current and valid Occupational Therapist license issued by the California Board of Occupational Therapy (CBOT).
• Current CPR certification.
• Valid driver’s license and access to reliable transportation.
• Ability to work independently and manage a caseload in a home health setting.
• Up to date Physical Health Exam and TB Testing
• Experience in home health setting preferred but NOT REQUIRED.
• Strong clinical judgment, communication, and time-management skills.
