Patient Responsibilities

- Pay for services according to agreed payment terms
Service Limitations
- Services are provided based on physician orders and medical necessity
- Emergency medical services are not provided
- We do not provide services outside our licensed scope of practice
- Treatment plans may be modified based on patient progress
- Services may be discontinued if safety concerns arise
- Some services may not be covered by insurance
Liability and Disclaimers
