About the Role

Key Responsibilities

• Conduct physical therapy evaluations to assess range of motion, strength, balance, endurance, gait, and functional mobility.
• Develop and implement customized treatment plans to address patient-specific goals such as safe ambulation, fall prevention, transfer training, and increased independence in movement.
• Reassess and modify therapy plans each visit based on patient progress and physical response to treatment.
• Instruct patients in therapeutic exercises and proper use of assistive devices like walkers, canes, or wheelchairs.
• Educate patients and their families on techniques for continued care, safe mobility, and home exercises between therapy sessions.
• Collaborate with physicians, nurses, and other therapists to ensure a coordinated care approach and make referrals when necessary.
• Review all referral documentation and patient history, including prior diagnoses, medications, and relevant reports, to guide therapy planning.
• Document each visit, evaluation, and treatment provided using the agency’s electronic system in a timely and accurate manner.
• Provide guidance to caregivers and families regarding patient needs, safety concerns, and expectations throughout the recovery process.

Requirements

• Graduate of an accredited Physical Therapy program (DPT, MPT, or BSPT).
• Current and valid Physical Therapist license issued by the State of California.
• Current CPR certification.
• Valid driver’s license and reliable transportation.
• Ability to work independently in patients’ homes and manage a flexible schedule.
• Up to date Physical Health Exam and TB Testing
• Experience in home health setting preferred but NOT REQUIRED.
• Strong clinical reasoning, communication, and documentation skills.
