About the Role

Key Responsibilities

• Administer therapeutic interventions as outlined by the supervising Physical Therapist, focusing on activities such as transfers, ambulation, muscle strengthening, balance training, and range-of-motion exercises.
• Observe and assess the effects of treatment during each visit, reporting progress and concerns to the supervising PT to adjust the plan of care for maximum benefit.
• Instruct patients in exercises, proper body mechanics, and mobility techniques designed to improve physical function and prevent injury.
• Educate patients and caregivers on the home exercise program and safety procedures for continued care after visits.
• Communicate and coordinate with the supervising PT, physicians, and other healthcare team members regarding patient status and needs.
• Review physician orders, previous diagnoses, and therapy notes as needed to understand patient history and guide treatment.
• Submit timely and accurate documentation of treatments, observations, and progress through the agency’s electronic system.
• Assist in educating patients and their families on what to expect during physical recovery and how to adapt daily routines to promote functional independence.

Requirements

• Graduate of an accredited Physical Therapist Assistant (PTA) program.
• Current and valid Physical Therapist Assistant license issued by the Physical Therapy Board of California (PTBC).
• Current CPR certification.
• Valid driver’s license and access to reliable transportation.
• Ability to work independently in a home health setting and manage a flexible schedule.
• Up to date Physical Health Exam and TB Testing
• Experience in home health setting preferred but NOT REQUIRED.
• Strong communication, observational, and patient-care skills.
